Withdrawal Requests

1. This form is used by students when they want to withdraw from a course.

2. Please note that there are two forms one for 9-12 and another for Y3-14 (Links below)

Year 13-14 Withdrawal Request

1. The questions asked are the following: First name, Last name, EMPLID, Grade, Semester, Course You Are Requesting to Withdraw From, Please indicate why you are requesting to withdraw.

2. Once this form is submitted, it needs to be opened in spread sheet.

3. The withdrawal needs to be completed and marked off on the spread sheet.

4. All documents need to be updated with the students that completed the withdrawal request form.
