Commitment Form

This form is sent out to Hero students to complete upon deciding to attend HOSTOS College.

1. Create a Google Form (use the template as a reference)

1. Share the form link to Ashante and Hero Liaison.

2. Collect form data (after the form is filled out by students).

3. Create acceptance letters using a template

4. Put the Hero/Hostos flags and acceptance letter into an envelope and mail them out.
